Project HELP (Health Education Literacy Project)
Description:
The Health Education Literacy Project (HELP) will work directly with 200 parents/caregivers and their infant/toddler/pre-school age children in a targeted effort to improve health outcomes for children and families of immigrants in Camden. The HELP initiative is a health literacy program designed to improve health outcomes for young children of immigrant families whose adult members have limited English proficiency and tend to have low levels of formal education in their native language. It will be delivered through a Health Literacy Curriculum that will be used with immigrant families of young children attending local day care centers. Project partners include the LEAP Academy University Charter School, LEAP Health Center/HOP Clinic, Respond Inc., Broadway Family Center, Mi Casita Day Care Center an El Centro Communal Borincano preschool.

Camden Coalition of Healthcare Providers
Description:
A citywide collaborative of healthcare providers is working to improve the quality and accessibility of Camden's healthcare system by developing a case management system targeting the city's highest needs patients, building web-based tools to improve communication between providers, and assisting medical offices with making practice management improvements.

Project Title:
Road to Recovery Program
Description:
Substance abuse treatment is enhanced with Intensive Outpatient Treatment provided in Spanish, targeting Latino residents of Camden County. The Hispanic Family Center of Southern New Jersey (HFC) will enhance its substance abuse treatment program through the addition of Intensive Outpatient Treatment. These services will serve Camden County residents with a majority of clients coming from the city of Camden. The project director is Elsa Candelario, Executive Director for Hispanic Family Center of Southern NJ.

Project H.O.P.E.'s Integrated Mental Health and Substance Abuse Treatment Model
Description:
Project HOPE is expanding its health services to the homeless population of Camden with a treatment model to provide services to homeless individuals suffering from co-occurring mental health and addiction.
