Zufall Health Center, a Federally Qualified Health Center, will develop and implement a two-year workforce redesign project that leverages recent renovations to its medical office site in Dover and builds on successes achieved through implementation of a Medicaid certified Electronic Health Record system and development of Patient Centered Medical Home (PCMH) care coordination teams. Areas of focus include integration of front office Patient Representatives into PCMH clinical care teams, introduction of group visits for patients with chronic conditions, and utilization of technology to improve effective communication with patients. Zufall Health Center will work with Primary Care Development Corporation, a nonprofit dedicated to expanding and transforming health care in underserved communities, to analyze its current operations and identify opportunities for improvement in patient care consistent with the Chronic Care Model, recognition as a PCMH, and Meaningful Use. The primary goal of this project is to optimize limited health care resources while improving patient outcomes and satisfaction.
