South Jersey Hospital is collaborating with ten organizational partners to conduct this community needs assessment in Cumberland and Salem counties.  Needs assessment activities include the creation of a secondary data profile, grass roots outreach via individual surveys, telephone surveys, multi-media surveys, a resource assessment, and focus groups for both counties. The key outcomes as a result of these project activities will include creation of a single database with all secondary statistical data, compilation of all primary data collection (i.e. surveys, focus groups, resource assessment) into a single report, prioritization of key issues, a Community Strategic Plan addressing these issues, and a final report that will be available online and in hard-copy. Through this assessment, SJH will continually strive to identify the key social and environmental issues affecting all vulnerable and disadvantaged populations of the community.
