Hunterdon Medical Center Foundation will implement its Streamlined Innovations for Better Access and Quality (SIBAQ) Project at Phillips Barber Family Health Center in Lambertville, NJ and Delaware Valley Family Health Center in Milford, NJ. to create an internal healthcare workforce development pipeline to increase skill sets of existing staff to support the practices’ shift to a team-based model. The project will build, test and refine a team approach to delivering care by introducing the Pre-Visit Planner role to each practice, by training existing Patient Service Representatives (PSRs) and Medical Assistants to identify care gaps in preparation for patient visits. SIBAQ will strengthen the practices’ workforce through training and team building, increase patient access through streamlined workflows and better care coordination and increase quality by better equipping providers to identify and close care gaps in high-risk patients.
