St. Francis Medical Center has partnered with the Trenton Health Team, Inc., an alliance between the City of Trenton, in conjunction with all three of the City’s hospitals and Trenton’s only Federally Qualified Health Center and PICO to forge a collaboration among community partners and Trenton residents and develop the Trenton Community Health Needs Assessment (TCHNA). The TCHNA is purposefully designed to engage the Trenton community in determining the City’s health needs priorities and create a health delivery system that reflects the distinct needs of City residents. The TCHNA will ultimately lay the foundation for a tightly coordinated, less costly and sustainable system of health care for the residents of Trenton.
